I've got exactly what you're after. This document is a few years old but an official publication from England Hockey. I got it when I did my level 2 coaching.

You do need a very reliable CH and super fit Insides for this formation to work IMHO, particularly when you're defending, due to the top heavy nature for the formation. I think it's particularly easy for the forwards (all 5 of them) to get separated from the rest of the unit, if players aren't really focused on disciplined, positional play. However, in terms of offensive play, you've got a lot of potential + options.
Also, everyone on the team has to be:
a) fit
b) understand the formation and do their jobs (so gaps don't open up)
However, I guess you could say that about every formation!
I've played this "W" formation before and enjoyed it.
I think though, more current thinking looks to 3-3-1-3. I certainly get the impression this is what gets played in europe.
