Coordination of substitutions

How best to coordinate subs when they are on the opposite side of the pitch? Without leaving holes in the formation how is it best to perform quick and coordinated subs.

First of all, subs have to realize that they need to sprint on/off the pitch to minimize the time needed to do the sub. In general I only sub the opposite side when the ball is off-pitch or a free hit is been awarded and in control of my team, so that we can set the pace. I warn the player that needs to come off, so he is prepared when the situation arrises and put the brake on fast play as soon as we have a free hit.  In the past I tried to move the player I wanted to sub to the correct side by interchanging players on the field, but that takes to much time and generates confusion in the lines.
