1 attacker, 4 midfielders
Defending team (in possession) set up in usual formation as if they are taking a goal kick (play out from the back etc).
Out of possession team will then need to block off all options as quickly as possible before the other team plays out from the back.
Occasionally the team in possession will mix things up and play long so they need to be prepared for this.
