How retrieve your previous form ?

i used to play the slog sweep very well but after few weeks i cannot play the shot very well .

Get in th nets and have someone throw down balls that you used to slog sweep and practice, practice and more practice until it comes back. If you have done it successfully before you can do it again. Form and shots can come and go there is no easy solution, only hard work.
